ABSTRACT

Objective To analyze the genetic subtypes of human immunodeficiency virus (HIV) and the prevalence of pretreatment drug resistance in the newly reported HIV-infected men in Guangxi. Methods The stratified random sampling method was employed to select the newly reported HIV-infected men aged≥50 years old in 14 cities of Guangxi from January to June in 2020.The pol gene of HIV-1 was amplified by nested reverse transcription polymerase chain reaction and then sequenced. The mutation sites associated with drug resistance and the degree of drug resistance were then analyzed. Results A total of 615 HIV-infected men were included in the study. The genetic subtypes of CRF01_AE,CRF07_BC,and CRF08_BC accounted for 57.4% (353/615),17.1% (105/615),and 22.4% (138/615),respectively.The mutations associated with the resistance to nucleoside reverse transcriptase inhibitors (NRTI),non-nucleoside reverse transcriptase inhibitors (NNRTI),and protease inhibitors occurred in 8 (1.3%),18 (2.9%),and 0 patients,respectively.M184V (0.7%) and K103N (1.8%) were the mutations with the highest occurrence rates for the resistance to NRTIs and NNRTIs,respectively.Twenty-two (3.6%) patients were resistant to at least one type of inhibitors.Specifically,4 (0.7%),14 (2.3%),4 (0.7%),and 0 patients were resistant to NRTIs,NNRTIs,both NRTIs and NNRTIs,and protease inhibitors, respectively. The pretreatment resistance to NNRTIs had much higher frequency than that to NRTIs (2.9% vs.1.3%;χ2=3.929,P=0.047).The prevalence of pretreatment resistance to lamivudine, zidovudine, tenofovir, abacavir, rilpivirine, efavirenz, nevirapine, and lopinavir/ritonavir was 0.8%, 0.3%, 0.7%, 1.0%, 1.3%, 2.8%, 2.9%, and 0, respectively. Conclusions CRF01_AE,CRF07_BC,and CRF08_BC are the three major strains of HIV-infected men≥50 years old newly reported in Guangxi,2020,and the pretreatment drug resistance demonstrates low prevalence.

ABSTRACT

Objective:To investigate the clinical effect of free anterolateral thigh perforator flap(ALTPF) in reconstruction of temporal scalp defect after blood circulation reconstruction surgery for moyamoya disease. Methods:From May 2020 to July 2022, 7 patients with scalp defect after revascularisation of moyamoya disease were treated in Department of Hand and Foot Microsurgery, Henan Provincial People’s Hospital (People’s Hospital of Zhengzhou University). The patients were 4 males and 3 females, aged 33-59 years old, at 43 years old in average. There were 5 defects in left tempus and 2 in right tempus. The sizes of scalp defect were 4.5 cm × 5.5 cm-7.5 cm × 9.5 cm. Debridement and VSD management were primarily performed. After wounds were stabilised, ultrasound location of perforator vessels of ALTPFs was performed. Having confirmed that the perforator vessels were suitable for the surgical requirements, flap transfers were then performed. The descending branch of the lateral femoral circumflex artery was end-to-side anastomosed with the superficial temporal artery, and the descending branch of the lateral femoral circumflex vein was end-to-end anastomosed with the superficial temporal vein. Postoperative follow-up was conducted through outpatient clinic visits, telephone and WeChat reviews. Appearance, texture of ALTPFs and the flap donor sites were observed in follow-ups. Comparisons of the changes of nervous system before and after surgery were made. Cognitive function of the patients was assessed with the Mini Mental State Examination (MMSE), together with the Activities of Daily Living (ADL) .Results:All 7 flaps survived. One flap had vascular compromise 6 hours after surgery, and was rectified after surgical intervention. All the patients were included in the postoperative follow-up for 7-33 (average 19) months. All flaps had good appearance with soft texture. There was no obvious difference in colour comparing with the skin around the recipient region. The donor sites healed well without hypertrophic scar. Examinations of nervous system of the patients were found the same as that before surgery. Using Manual Muscle Testing (MMT), the average limb muscle strength of the patients was 4 before surgery and 4 after surgery, without change; Using the Ashworth assessment scale, the average preoperative and postoperative limb muscle tension in this group of patients was 1, without change; The Berg balance scale was used to evaluate the patient's balance function, with an average score of 42 before surgery and 42 after surgery, without any changes; There was no change in limb sensation before and after surgery; Using the MMSE, the average preoperative score and postoperative score of this group of patients were 25 points, without any change. Using the modified Barthel index scoring standard, the average preoperative score for this group of patients was 75 points, and the average postoperative score was 79 points, and the ADL of the patient had improved to various levels. Conclusion:Reconstruction of scalp defect with free ALTPF after revascularisation of moyamoya disease has obvious advantages, such as it closes the wound quickly, prevents infection and achieves a good appearance. This surgical procedure can produce a good clinical effect.

ABSTRACT

Objective:To investigate the surgical method and clinical effect of one-stage repair of soft tissue defects of dorsum of hand and multiple fingers by using the polyfoliate conjoined anterolateral thigh perforator flap (ALTP) .Methods:From November, 2015 to September, 2019, 9 cases of soft tissue defects of dorsum of hand and multiple fingers were treated, including 8 males and 1 female. The average age was 32 years old. Causes of injury: 4 cases of thermal injury, 3 cases of strangulation injury, and 2 cases of traffic accident injury. Location of injury: 7 cases of right hand, 2 cases of left hand. Three cases of dorsum of hand with 1st-4th fingers, 3 cases with 2nd-5th fingers, 2 cases with 2nd-4th fingers, and 1 case with 3rd-5th fingers; All with bone, joint and tendon exposure. There were 6 cases with fracture or joint dislocation. All of them had no obvious defect of bone and joint. VSD was used to cover them after debridement. According to the shape and size of the wound, all flaps were designed to repair the soft tissue defect as the polyfoliate conjoined ALTP. The flap size was 7 cm×13 cm-12 cm×17 cm. Regular outpatient, telephone and Wechat follow-up were carried out to evaluate the flap survival, appearance, color elasticity, donor scar, sensory recovery, finger function recovery and patient satisfaction. Results:Nine cases of flap survived well. The average followed-up time was 12 (6-18) months. The postoperative followed-up revealed satisfactory shapes of the flaps, protective touch and recovery of functions. The donor site had good healing and no effect on function. In the last followed-up, according to the trial criteria for function evaluation of upper limbs by Chinese Society of Hand Surgery, 5 cases were excellent, 3 cases were good, and 1 case was fair. The hand function recovered well and returned to work and daily life on average 6 weeks after operation. Conclusion:The polyfoliate conjoined ALTP can repair the soft tissue defect of hand dorsum with multiple fingers at one stage, and the flap is in good shape. There is no need to divide the fingers. It is one of the ideal methods to repair the soft tissue defect of hand dorsum.

ABSTRACT

Respiratory syncytial virus (RSV) is an RNA virus, which belongs to the paramyxoviridae family, and is transmitted by air droplets and close contact and the main pathogen causing acute lower respiratory tract infection in infants, the elderly and immunocompromised individuals.Although there have been studies on the prevention and treatment of RSV drugs and RSV infection in patients, many medical demands have not been met.And there is no specific antiviral therapy. The only two drugs approved to be applied in RSV prevention and treatment are perizol and ribavirin. However, the former must be used prophylactically, and primarily in high-risk children, while the latter is less effective, and some children even suffer from airway spasm. Therefore, it is urgent to propose new methods for prevention and treatment of RSV.In recent years, traditional Chinese medicine(TCM) has shown a good anti-RSV effect, with a fewer side effects, less resistance to drugs and broad-spectrum antiviral advantage. There are also newly developed biological and chemical anti-RSV drugs. Some new drugs have shown a good efficacy, with an extended half-life and reduced costs, such as fusion inhibitors, monoclonal antibodies. This paper reviews the research progress of anti-RSV drugs in the aspects of TCM, biological drug and chemical drugs, laying a foundation for the development of new anti-RSV drugs and the formulation of new therapeutic strategies.

ABSTRACT

Objective:To study the protective effect and mechanism of Chaenomelis Fructus alcohol extract (CFE) on the synovium of rheumatoid arthritis (RA). Method:Sixty male SD rats were randomly divided into normal group and model group. RA model was made by injection of complete Freund's adjuvant, and then was randomly divided into model group, CFE low, medium and high dose group and Tripterygium glycoside group according to the inflammatory score. The CFE groups (0.15,0.30,0.60 g·kg-1·d-1) had intragastric administration once a day for 30 d after the model establishment. The blank control group and the model group were given the same volume saline water by gavage. After all the drugs were given, the blood, joint tissues and synovium tissue of rats were collected. The levels of interleukin-1β (IL-1β), interleukin-6 (IL-6), interleukin-10 (IL-10) and tumor necrosis factor (TNF)-α in serum were detected by enzyme linked immunosorbent assay (ELISA), the pathological changes of synovium were observed by hematoxylin eosin (HE) staining, and the expressions of B-cell lymphoma-2 (Bcl-2),Bcl-2 associated X protein (Bax) and death factor (Fas) in joints were detected by Western blot. Result:Compared with normal group, the swelling degree and inflammation index of rats' feet in model group increased significantly, the levels of inflammatory factors IL-1β, IL-6, TNF-α in serum increased (P<0.01), anti-inflammatory factor IL-10 decreased (P<0.01), the protein expression of Bax, Fas and Bcl-2 increased, and the statistical results of Bcl-2 showed significant difference (P<0.05). Compared with model group, the swelling degree and inflammatory index of the plantar of RA rats were improved in the middle and high dose groups of CFE (P<0.01), the pathological changes such as synovial tissue hyperplasia and inflammatory cell infiltration were reduced in each dose group, and the levels of IL-1β, IL-6 and TNF-α in serum were reduced (P<0.05), anti inflammatory factor IL-10 increased (P<0.05), the disorder of inflammatory cytokine in the model was corrected, Bax, Fas expression increased, Bcl-2 protein expression decreased (P<0.01). Conclusion:CFE can reduce the degree of inflammation in RA joint and has obvious anti RA effects, which may be related to the apoptosis of synoviocytes induced by CFE.

ABSTRACT

Objective@#To investigate the factors associated with the anastomotic leakage after anterior resection in rectal cancer.@*Methods@#From January 2014 to January 2017 471 patients underwent Dixon procedure for rectal cancer in The Affiliated Hospital of Qingdao University. The data of those patients was collected and reviewed retrospectively. Inclusion criteria included: 1) rectal cancer confirmed by preoperative electron colonoscopy; 2) the standard of total mesorectal excision followed by the surgeon during the surgery; and 3) elective surgery. Exclusion criteria included multi-primary rectal cancer, secondary surgery for tumor recurrence, palliative surgery, Miles procedure, Hartmann procedure, hormone drugs used, presence of rheumatic and immune diseases, and distant metastasis of rectal cancer. The variables, including demograpic characteristics, ASA score, diabetes mellitus, preoperative radiochemotherapy, histopathologic grade, pathological T stage, laparoscopic or open surgery, distance of the tumor from the anal verge ≤5 cm, were analyzed to identify the risk factors for anastomotic leakage.@*Results@#Of 471 patients, 285 and 186 were men and women, respectively, with a mean age of 61 years (range, 31-92) years. Symptomatic clinically anastomotic leakage occurred in 31 patients (6.6%, 31/471) after Dixon procedure for rectal cancer. On univariate analysis, the occurrence of anastomotic leakage was associated with diabetes (χ2 = 10.972, P = 0.001) , serum albumin level < 35 g/L (χ2 = 9.784, P = 0.002) , neoadjuvant chemoradiotherapy (χ2 = 6.867, P = 0.009) , distance ≤5 cm between the tumor and anal edge (χ2 = 5.993, P = 0.014) , preventive colostomy (χ2 = 5.630, P = 0.018) , and the use of double-perfusion cannula for abdominal flushing (χ2 = 4.232, P = 0.040) . Multivariate analysis revealed that diabetes (OR = 3.632, 95%CI: 1.620-8.145, P = 0.002) , neoadjuvant chemoradiotherapy (OR = 3.177, 95%CI: 1.283-7.867, P = 0.012) and distance ≤5 cm between the tumor and anal edge (OR = 2.444, 95%CI: 1.172 - 5.059, P = 0.017) were independent risk factors for anastomotic leakage, while preventive colostomy (OR = 0.138, 95%CI: 0.056 - 0.345, P = 0.000) and the use of double-perfusion cannula for abdominal flushing (OR = 0.223, 95%CI: 0.086 - 0.575, P = 0.002) were independent protective factors for anastomotic leakage.@*Conclusions@#For patients with rectal cancer with diabetes, undergoing neoadjuvant chemoradiotherapy, or distance ≤5 cm between the tumor and anal edge, anastomotic leakage after anterior resection of rectal cancer must be paid attention. When necessary, preventive colostomy or use of double-perfusion cannula for abdominal flushing should be considered.

ABSTRACT

Objective: To observe the effect of electroacupuncture (EA) at Huantiao (GB 30) and Weizhong (BL 40) on thigmesthesia, gait function, and expression levels of serum immunoglobulin G (IgG) and immunoglobulin M (IgM) in rabbits with lumbar intervertebral disc herniation (LIDH). Methods: Forty healthy New Zealand rabbits were randomly divided into a blank control group, a model group, an EA at acupoint group and an EA at non-acupoint group, with 10 rabbits in each group. The LIDH pathological model of rabbit was established using the self-made LIDH model maker. The thigmesthesia and gait function of rabbits were recorded by Siegal method. The serum IgG and IgM expression levels were detected by enzyme-linked immunosorbent assay. Results: EA at Huantiao (GB 30) and Weizhong (BL 40) could improve the clinical symptoms of thigmesthesia and gait function, and inhibit the expressions of serum IgG and IgM in the LIDH rabbits, which were significantly different compared with those in the model group and EA at non-acupoint group. Conclusion: EA at Huantiao (GB 30) and Weizhong (BL 40) can improve the clinical symptoms of LIDH rabbits, which is associated with inhibition of the serum IgG and IgM expressions and reduction of the immunoinflammatory factor release. This may be one of the mechanisms of EA at Huantiao (GB 30) and Weizhong (BL 40) in the treatment of LIDH.

ABSTRACT

The chimeric antigen receptor (CAR) T cell therapy has gradually became a new trend in the treatment of refractory and relapsed hematologic malignancies by developing for 30 years. With the exciting development of genetic engineering, CAR-T technology has subjected to 4 generations of innovation. Structure of CAR-T started from a single signal molecule to 2 or more than 2 co-stimulatory molecules, and then coding the CAR gene or promoter. CAR-T can specifically recognize tumor antigens, and does not be restricted by major histocompatibility complex (MHC), thus making a breakthrough in clinical treatment. In this review, the history, structure and mechanism of action of CAR-T, as well as the current status and challenges of CAR-T immunotherapy in acute lymphoblastic leukemia, acute myeloid leukemia, chronic myeloid leukemia and multiple myeloma are summarized.

ABSTRACT

<p><b>OBJECTIVE</b>To provide a feasible and cost-effective next-generation sequencing (NGS) method for accurate identification of viral pathogens in clinical specimens, because enormous limitations impede the clinical use of common NGS, such as high cost, complicated procedures, tremendous data analysis, and high background noise in clinical samples.</p><p><b>METHODS</b>Viruses from cell culture materials or clinical specimens were identified following an improved NGS procedure: reduction of background noise by sample preprocessing, viral enrichment by barcoded oligonucleotide (random hexamer or non-ribosomal hexanucleotide) primer-based amplification, fragmentation-free library construction and sequencing of one-tube mixtures, as well as rapid data analysis using an in-house pipeline.</p><p><b>RESULTS</b>NGS data demonstrated that both barcoded primer sets were useful to simultaneously capture multiple viral pathogens in cell culture materials or clinical specimens and verified that hexanucleotide primers captured as many viral sequences as hexamers did. Moreover, direct testing of clinical specimens using this improved hexanucleotide primer-based NGS approach provided further detailed genotypes of enteroviruses causing hand, foot, and mouth disease (HFMD) and identified other potential viruses or differentiated misdiagnosis events.</p><p><b>CONCLUSION</b>The improved barcoded oligonucleotide primer-based NGS approach is simplified, time saving, cost effective, and appropriate for direct identification of viral pathogens in clinical practice.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical characteristics of different ages of children with acute exacerbation of bronchial asthma.</p><p><b>METHODS</b>The clinical data of 118 children with an acute exacerbation of bronchial asthma between June 2012 and June 2015 were retrospectively analyzed. These patients were classified into infant group (<3 years old), preschool group (3-6 years old), and school-age group (6-14 years old) to compare their clinical characteristics.</p><p><b>RESULTS</b>The infant group had the highest rate of pneumonia, the highest rate of hospital use of antibacterial agents, the highest hospital costs, and the longest length of hospital stay, followed by the preschool group and the school-age group (P<0.05). For the maintenance treatment of asthma, the rate of use of inhaled corticosteroids was highest in the school-age group (70%), followed by the preschool group (50% )and the infant group (38%) (P<0.05).</p><p><b>CONCLUSIONS</b>The clinical characteristics vary between different ages of children with acute exacerbation of bronchial asthma: the children less than 3 years old have a higher rate of pneumonia, a higher rate of use of antibacterial agents, higher hospital costs, a longer length of hospital stay, and a lower rate of standard treatment.</p>

ABSTRACT

Objective: To establish an HPLC-fingerprint method for evaluating the quality of the root of Polygonum multiflorum from different habitats in China. Methods: A separation was performed on an Agilent extend C18 chromatographic column (250 mm × 4.6 mm, 5 μm) with gradient elution. The mobile phase consisted of acetonitrile-formic acid water solution (0.1%). And the mobile phase was at flow rate of 1.0 mL/min. The detection wavelength was at 280 nm. HPLC fingerprint chromatograms of 16 batches of crude material of the roots of P. multiflorum were analyzed. Chemometric methods including principle component analysis (PCA), and clustering analysis (CA) were used through the software of Matlab for common peaks. And the content of 2,3,5,4'-tetrahydroxy stilbene-2-O-β-D-glucoside was determined by HPLC. Results: The similarities of the 16 batches of samples were between 0.936 and 0.998. This showed that the quality of the roots of P. multiflorum from different habitats in China was stable. Eighteen common peaks were identified by HPLC/Q-TOF/MS. The results of FCA and CA were consistent with similarity evaluation. The content of 2,3,5,4'-tetrahydroxy stilbene-2-O-β-D-glucoside was consistent with principle component loading factors. Conclusion: The method is simple reliable, and it can provide a standard and guidance for the quality control of the roots of P. multiflorum.

ABSTRACT

Objective To investigate the changes of electrode impedance, threshold(THR),maximum comfortable level(MCL),and dynamic range(DR),providing the basis for debugging after operation in congenital deaf children. Methods A respective study was carried out on 20 cases of congenital deaf children who were implanted with cochlear Med?EL SONATAti100. The measurements and changing ranges of electrode imped?ances,THR,MCL and dynamic range during the operation and at postoperative one month, three months, and six months were statistically analyzed. Results Electrode impedances were the lowest during the operation and the highest at one month after operation(P0.05). Conclusion The electrode impedances varied due to different electrode posi?tion. The vales of THR,MCL and DR were unrelated with electrode impedance. DR was gradually increasing,MCL reached steady state earlier than THR. It is important that THR should be debugged at three months after operation and MCL should be debugged at six months.
